Ketone Test Strips: What They Measure and When to Switch to Blood

Urine ketone strips detect acetoacetate, not the ketone your body runs on. Here is what that means for keto dieters, fasters, and people with diabetes.

Ketone strips are cheap, fast, and sold everywhere. They are also the most misunderstood kit in the category. Most buyers do not know what the strip measures. That gap explains almost every confused review.

Three ketones, one pad

When the body burns fat for fuel, the liver produces three compounds called ketone bodies:

  • Acetoacetate. The first one made.
  • Beta hydroxybutyrate (BHB). Made from acetoacetate. The dominant ketone in the blood, and the one cells actually use for energy.
  • Acetone. A byproduct. Leaves through the breath.

The urine strip pad uses a nitroprusside reaction. It reacts with acetoacetate and, weakly, with acetone. It does not react with BHB at all.

Early in ketosis, the body makes a lot of acetoacetate and spills the excess into urine. The strip turns purple. Over days to weeks, the body gets better at converting acetoacetate to BHB and better at using ketones instead of wasting them. Blood BHB stays high. Urine acetoacetate drops. The strip fades to trace or negative.

The person is still in ketosis. The strip just cannot see it anymore.

Who the strips work for

People starting a ketogenic diet. The first two to four weeks are exactly when urine strips work best. They confirm that carbohydrate restriction is producing ketones. That feedback is useful and cheap.

People doing short fasts. A one to three day fast pushes acetoacetate up fast. Strips track it.

People with type 1 diabetes checking for ketoacidosis. This is a medical use, not a diet use. High ketones plus high blood sugar plus feeling unwell is an emergency. Urine strips can flag it. Blood meters are better because they read BHB, which rises first and highest in ketoacidosis. Anyone managing type 1 diabetes should follow their clinician's guidance on which to keep at home.

Who should switch to a blood meter

Anyone in ketosis longer than about a month who wants an actual number. A blood ketone meter reads BHB in millimoles per liter. Nutritional ketosis is usually defined as 0.5 to 3.0 mmol/L. The meter costs more up front and each strip costs several times what a urine strip does. It also answers the question the urine strip stopped answering.

If you just want a yes or no and do not mind that the yes fades over time, urine strips are fine.

Reading the pad

The chart runs from negative through trace, small, moderate, and large. Some bottles print mg/dL values: 5, 15, 40, 80, 160. The color goes from beige to deep purple.

The read window is short, usually 15 to 40 seconds. It is one of the fastest pads on any strip. Read late and the color is darker than the true value. Use a timer.

Hydration changes the reading a lot. Concentrated morning urine reads higher. Dilute afternoon urine reads lower. Compare readings taken at the same time of day.

Things that cause false readings

  • False positives: some medications containing sulfhydryl groups, such as captopril and mesna. Highly pigmented urine.
  • False negatives: old or humid strips. The nitroprusside reagent is sensitive to moisture. A bottle left open in a bathroom loses sensitivity in weeks.
  • Confusing negatives: long term ketosis, as explained above. The strip is not broken.

Buying notes

  • Buy the smallest count that covers your first month. After that you will either stop testing or move to blood.
  • Strips in a bottle with a desiccant cap are fine if you close it fast. Individually wrapped strips last longer but cost more.
  • Combination strips that read ketones and glucose together are useful for people with diabetes. For diet tracking, the glucose pad is dead weight.
  • Ignore marketing about "keto specific" strips. The chemistry is the same nitroprusside pad on every brand. Price and expiration date are the differences that matter.
