Glucose in Urine: What a Positive Pad Means
Sugar in urine usually means blood sugar has gone above the kidney's threshold. Sometimes it means something else. What the glucose pad detects and what to do next.
The glucose pad is the oldest test on the urine strip. Before blood glucose meters existed, urine sugar was how diabetes was tracked at home. It is a crude tool for that purpose now, but a positive pad still carries a clear message.
Why glucose is normally absent
Blood constantly carries glucose. The kidneys filter it out into urine and then reabsorb nearly all of it back into the blood. The reabsorption system has a capacity limit, called the renal threshold. For most people it sits around a blood glucose of 180 mg/dL. Below that, urine has no detectable glucose. Above it, the excess spills over.
So a positive glucose pad usually means one thing: blood sugar was above roughly 180 at some point while that urine was being made.
What the pad detects
The pad uses an enzyme, glucose oxidase, that reacts only with glucose. Other sugars, including fructose and lactose, do not trigger it. The color shifts from blue green through green to brown as the concentration rises. Readings are reported as negative, trace, or in mg/dL steps, commonly 100, 250, 500, 1000, and 2000.
The reaction is fast. Most strips read glucose at 30 seconds. Reading late overstates it.
Causes of a positive result
High blood sugar. Undiagnosed or poorly controlled diabetes is the main cause. A positive pad in someone not known to have diabetes is a reason to get a blood test, either a fasting glucose or an A1C. See diabetes screening explained.
A large sugar load. A very large sweet meal can push blood sugar past the threshold briefly in some people. If the pad is positive after a dessert and negative the next morning, that is the likely story. Repeat on a fasting morning.
Pregnancy. The renal threshold drops in pregnancy, so glucose appears in urine at lower blood sugar levels. A trace result in pregnancy is common and is followed up with blood testing for gestational diabetes, not with panic.
SGLT2 inhibitor medications. Drugs like empagliflozin, dapagliflozin, and canagliflozin work by making the kidneys excrete glucose on purpose. Anyone taking one will have a strongly positive glucose pad every time. That is the drug working, not a problem.
Kidney tubule problems. Rarely, the reabsorption system itself is impaired and glucose leaks despite normal blood sugar. This is called renal glycosuria. It is uncommon and usually harmless, but it is diagnosed by a clinician after the more common causes are excluded.
Causes of a false result
False negatives: high dose vitamin C is the classic one. It interferes with the pad chemistry and can hide real glucose. Old or humid strips lose sensitivity. Very dilute urine can push a real positive below the detection threshold.
False positives: rare with modern strips. Some cleaning agents, bleach or peroxide residue in a reused cup, can cause a color change. Use a clean, dry container.
What a home reading cannot tell you
It cannot tell you your blood sugar. Urine is a lagging, pooled signal. The urine you pass now was made over the last few hours. A positive pad means blood sugar was high at some point in that window. It does not say how high or whether it still is.
That is why blood glucose meters replaced urine strips for diabetes management decades ago. A meter gives the number now. Anyone managing diabetes should use a meter or a continuous monitor and treat the urine pad as a curiosity.
What to do with a positive
If you are not known to have diabetes and the pad is positive on a fasting morning, book a blood test. Do not wait for a second strip to confirm it. The blood test is the confirmation.
If you are pregnant, tell your clinician at the next visit. It will be checked.
If you take an SGLT2 inhibitor, ignore the pad.
If it was positive once after a sugary meal and negative on a fasting repeat, note it and move on. If that pattern keeps happening, mention it. It can be an early sign that the body is handling sugar loads less well than it should.
